Member Portal Registration Form

  • Create a username between 8 and 12 characters in length. You can use lower case, upper case or numbers.

  • First Name

  • Last Name

  • Enter your date of birth in the format: mm-dd-yyyy

  • Enter your phone number above in case we need to contact you for verification purposes.

  • Create a password between 8 and 12 characters in length. You can use lower case, upper case, numbers, and/or special characters.

  • Band Member
 

    • Please note: Check your junk mail for resetting password/registration emails.
     
    Your registration needs to have accurate information that matches our Membership records.
